Dismissal Permissions

My child is allowed to walk or ride home on their own and I hereby grant permission to Chess Academy to dismiss them after class.

I grant Chess Academy permission to dismiss my child after class if a nominated authorised person is not present to collect them at end of class and release Chess Academy from any duty of care in such instances.

I do not give permission to dismiss my child after class if there is no one to present to pick them up. I understand that late pick-up fees as high as $100 per 15 minutes apply if my child is picked up late.
